Resident of practice catchment area and proof of Photo Identity and address.

Registration Process

Step 1: Complete Pre-registration Questionnaire

Step 2: Email "Proof of Photo Identity & address" to info.e12medicalcentre@nhs.net with Subject: New Patient Registration

Please make sure, you provide a correct mobile number. Practice to arrange a New Patient Registration appointment

If you don’t receive a call from the practice to arrange a New Patient Registration appointment within 72 hours of sending your email, please contact the GP practice.
