- House or Motor Insurance Certificate,
- Tenancy Agreement,
- Pension Notification Letter,
- HMRC Tax Notification Letter,
- University or College Letter or
- UK Home Office Letter
Registration Process

- Subject: New Patient Registration
- Attach proof of Photo Identity & address with your name, date of birth & contact number.
Please make sure, you provide correct:
Using Patient Access (after completion of registration), you can:
- Book appointment
- Order Repeat Medications
- See your Medical Records
If you don’t receive a call from the practice to arrange a New Patient Registration appointment within 72 hours of sending your email, please contact the GP practice.
Our practice policy has always been to have health check first at the time of registration either with our nurse or with our healthcare assistant. This enables us to update your records and clinicians to have better understanding of your health status.
New Patient Health Check: Mandatory to complete your GP registration.
Following checks may be required during the check
- Chlamydia Test (aged 16 to 24 years) - Download a Leaflet (PDF, 257KB)
- HIV Test - All new patients
- TB (Tuberculosis) Test (aged 16 to 35 years and living in the UK for less than 5 years)
- NHS Health Check (40 to 74 years and No Chronic disease)
